Registration Form

Registration Fee — A registration fee (payable in US dollars) is due at the time of registration. This investment in training includes all materials, handouts, and educational sessions. It does not include travel or hotel accommodations. This payment secures your registration for the desired program you wish to attend. You may use a credit card to hold your confirmation until a check is issued. Payment must be received prior to the workshop, or your registration is subject to cancellation.

Cancellation Policy — 100% refund if a written cancellation notice is received two weeks prior to the workshop. A $250 administrative fee will be charged for a cancellation/reschedules of less than 2 weeks. No refund will be given for cancellation the day before the commencement of the workshop or after. You may substitute an individual up to 24 hours prior to the commencement of class. This policy is strictly enforced due to the limited spaces available and guaranteed hotel contracts. Thank you for your understanding.
